What is Health Central?

 

The approach and strategy for the Health Central program revolves around preventative medical care and chronic disease management.  Connectinc believes that healthy lifestyles and access to reliable, consistent health care services are essential to increasing an individual’s self-sufficiency and contribution to the community.  Connectinc has already begun to address this need by piloting a prescription assistance program, called Health Central. 

 

Who is eligible?

 

The target population to be served by Health Central is dislocated workers who do not have prescription insurance coverage and whose total household income is low. 

 

The prescription care assistance program is currently being piloted in seven North Carolina counties which consist of Harnett, Hoke, Scotland, Richmond, Montgomery, Moore, and Lee counties. The pilot phase was launched on January 24, 2007 by Connectinc in collaboration with Sandhills Community Care Network and Mid-Carolina Physician Organization. 

 

When requested funding is secured, this program will be provided to dislocated workers in other counties. 

 

 

How does Health Central work?

 

  • The potential participant simply calls toll free at 888-927-3230 to file an application.
  • An application is completed by a Connectinc representative based on the information provided over the phone
  • The Connectinc representative will be able to immediately notify the worker if the medication(s) is available for free or at a reduced cost
  • The worker will be required to mail Connectinc required financial information to support the financial data collected by phone.
  • Once the financial data is received, the completed application package will be mailed to the worker for required signatures from the worker and his or her physician.
  • Based on the pharmaceutical manufacturer requirements, the medication will be mailed directly to the worker or the physician’s office. 
  • Once the medication is received and dispensed, Connectinc will update its records which will prompt a representative to contact the worker when it is time to reorder the medication.

 

 

The Health Central program will help dislocated workers learn a healthier way of thinking and living and allow their income to provide for necessities for them and their families. Health Central is yet another way that Connectinc helps families get connected.
